P3 partners Age UK in ‘social prescribing’ service

6 August 2018

P3, in partnership with Age UK Milton Keynes, has received funding from the Department of Health to expand its social prescribing offer to residents of Milton Keynes.

The additional and continued funding was granted after a successful 12-month pilot ‘High Impact User Service’ delivered by P3. Over the past year, this service has been supporting people who frequently used the emergency services by addressing complex needs such as social isolation, substance misuse and financial issues, and linking people into appropriate local services.

P3 were able to demonstrate a reduction in 999 calls by 37%, a reduction in A&E attendance by 29%, and a reduction in the need for emergency hospital admissions by 22%, saving considerable cost to local health services.

The expanded Milton Keynes P3 Health Service will be delivered as part of the ‘LiveLife’ Milton Keynes programme alongside Age UK who will handle referrals as a Single Point of Access. The service will be aimed at individuals who require a “social prescription” of social, emotional, and practical support to improve their wellbeing, whilst supporting them to reduce their usage of emergency services such as A&E and 999.

The service will officially launch in October 2018 and will be funded by the Department of Health jointly with Milton Keynes CCG.
