Wolverhampton Hospital Discharge Team

What we do

Our team works with staff from Penn Hospital and New Cross Hospital in Wolverhampton to identify and support patients who are homeless or at risk of becoming homeless during their stay in hospital.

Who we support

We can support you if:

You are over 18
You are a current in-patient at a participating hospital
You are homeless or at risk of homelessness upon hospital discharge

How we can help

We believe anyone being discharged from hospital should have somewhere safe to live and recover. We work with patients whilst they are still in hospital to ensure that they have suitable accommodation and that they have support in place when they are discharged to enable a successful recovery.
