What we do

We support people aged 18 and over who are experiencing or at risk of homelessness while in hospital. We help them to find a place to live once they are discharged and provide housing support during their recovery journey.

Who we support

We support people aged 18 and over who are currently an in-patient at a participating hospital in the Black Country and who are at risk of homelessness once they are discharged.

How to get help

I’m a member of hospital staff

  • Referrals can be made through the single-point-of-access from the hospital ward.

P3 Black Country Hospital Discharge Service works with hospital staff to identify and support patients who are homeless or at risk of homelessness during their stay.

We believe that everyone leaving hospital should have a safe place to live and recover, so we work with patients before they are discharged, ensuring they have suitable accommodation and the right support in place for a successful recovery.
