Milton Keynes P3 health Service

Alternative social, emotional and practical support for people who frequently use health services

This service will be delivered using investment from the Department of Health 'social prescribing' scheme, which allows organisations like P3 to provide social, emotional and practical support in order to improve people’s quality of life, and help them access the most appropriate healthcare resources.

The P3 Health Service will be delivered as part of the wider ‘Live Life’ Milton Keynes programme. P3 has partnered with Age UK Milton Keynes, with P3 providing support to individuals with medium to high needs, and Age UK Milton Keynes supporting those who need shorter term or one-off support.

The P3 Health team, and the wider “Live Life” Milton Keynes provision, is independent of the emergency services, the police and the NHS.

Who we support

You can be referred to this P3 service if you are aged 18+ and:

  • A frequent user of emergency or other health services
  • Would benefit from a comprehensive and longer period of emotional and practical support to improve your life
  • Experiencing feelings of social isolation, mental ill-health, substance misuse, or other life challenges

If you feel you would benefit from a service like this, but simply require a small amount of support, information, advice, or guidance, then Live Life Milton Keynes may still be able to help.

How to get help

Age UK Milton Keynes act as the single point of access for Live Life Milton Keynes provision, including the P3 Health team. You can contact the scheme for yourself (or on behalf of someone else if you have their permission), by calling 01908 973720 and asking for the Live Life Milton Keynes service.
