Important Notice

We are currently experiencing a high demand for the Social Prescribing Service and we are working through a waiting list as quickly as possible, please be mindful when referring.

We work closely with AGE UK who also provide a Social Prescribing Service and you can also speak to your GP Surgery about their in house Social Prescribing Service.

 

Within P3 Livelife we have two individual support services:

Social Prescribing Service: Professional and self-referrals
High Intensity User Service: Professional referrals only

*If completing a referral on someone else’s behalf, you will need to obtain verbal consent prior to making the referral.

Social Prescribing Service

This service works alongside people who may be feeling isolated and need some help to manage practical tasks and access the right support. We can link you into services to support with a range of needs including:

  • Loneliness, social isolation and getting back into the community
  • Mental and physical health
  • Access to support with finances, benefits and debt
  • Access to support for addiction
  • Housing advice
  • Relationships and staying safe

P3 can provide one-to-one holistic and practical support for up to 12 weeks. A friendly link worker will meet you in your home, out in the community and/or speak to you over the phone to create a support plan to suit you. They will be there every step of the way to help you reach your goals. 

We can support you if:

  • You are aged 18+ and:
  • Live within the Milton Keynes Local Authority area
  • You are registered with a Milton Keynes GP

High Intensity User Service

(NHS or Local Authority referrals only)

This service works with people who use health and other acute, emergency services much more frequently than would be normally expected.

We can provide a comprehensive package of holistic and practical support in a person’s home, out in the community or remotely by phone to help reduce calls and attendances.

One of our experienced and friendly link workers will help create a support plan designed to suit the individual to help reach their goals, whether they are experiencing feelings of social isolation, mental ill-health, addiction or other life challenges.

We aim to provide a multi-agency approach to support and will ‘link’ into the appropriate services where necessary.

We can support if:

  • The person is a frequent user of emergency or other health services.
  • The referrer obtains verbal consent from the person prior to referral
  • The referrer has an NHS or Local Authority email address
  • The person is aged 18+ and:
    • Lives within the Milton Keynes Local Authority Area
    • Is registered with a Milton Keynes GP

Evidence of activity three months prior to referral will need to be provided for any of the following: A&E Attendances, Hospital Emergency Admissions, South Central Ambulance Service (SCAS) 999 Calls.
